Fill out an adult intake form or a child intake form and guardian authorization.

Once the intake form is submitted a member of our Intake team will reach out to you with next steps, including confirming insurance, finding the right provider for you, and scheduling an initial appointment.

Arrive at our Needham, Norwell, or Hanover office, or log online for Telehealth appointments, to meet your provider in person.
